Early Intervention Services
Tender Age PT provides therapies to children birth to 21 years of age. The therapies
are provided in our office, at the child’s home or school. The staff at Tender Age
PT are dedicated professionals providing quality services to children and their
families. It is our goal for every child to reach their maximum potential.
Early Intervention Program
Tender Age PT is an approved Suffolk County provider of Early Intervention services.
Children from birth to 3 years old with special needs are eligible to receive services.
Eligibility for services is determined by an interdisciplinary evaluation. A child
will receive services if the evaluation shows a significant developmental delay.
If you suspect your child has a developmental delay, please contact the Suffolk County
Early Intervention Program at the following number: 853-3100. The evaluation and
the therapies are provided to eligible children at no financial cost to the families.
Preschool Related Services
Tender Age PT contracts with Suffolk County to provide Preschool Related Services
and Special Education Itinerant Services to eligible children ages 3 – 5 years.
These services can be provided in your home, our office, day-care or at preschool.
Eligibility for the services is determined by the Committee on Preschool Education
(CPSE) of your home school district.
School Age
Services are provided to children 5 to 21 years of age in school, at home or at
our office. Services to be provided are determined by the Committee of Special Education
(CSE) of your home school district. We have contracts with a number of school districts.
